How to Configure a DEC EtherWORKS II Turbo NIC

The DEC EtherWORKS II Turbo NICs are half-size 16-bit ISA cards based on the AMD 7990 LANCE Ethernet Controller. Each of the boards has a 12 pin DIP switch which has the same meaning for all EtherWORKS II Turbo NICs. The rest of this page documents those switch settings.
